Coaching Agreement

Thank you for choosing Anita Birdwell Financial Coach, LLC as a part of your financial plan. I look forward to serving you and ask that you review the information below to help guide our working relationship.

Scheduling Appointments: Coaching appointments are scheduled from 9:00 a.m. to 6:00 p.m. CST,

but I can occasionally meet earlier in the morning or later in the evening. I understand that your time

and resources are extremely valuable, and in my efforts to serve you best, I ask that you make the

necessary sacrifices and commitments to meet during the times listed above. Appointments can be

conducted at a neutral site, over the phone or online (i.e. Zoom).

Coaching Service: I assume the responsibility of understanding/evaluating your current needs, goals

and resources in order to develop a strategy to meet your objectives. However, in order to make your

plan work, I expect that you will come prepared, take full responsibility for your plan and its

implementation, and contact me when you need additional advice and direction.

Paperwork: You are asked to complete preliminary paperwork to better prepare your coach. I ask that

these forms be filled out to the very best of your knowledge and ability, prior to your scheduled

appointment. You assume all responsibility for providing accurate and complete materials,

documentation, and facts upon which my coaching may be based.

Spousal Participation: If you are married, both spouses must participate in all coaching sessions.

Refund Policy: I do not offer refunds. All sales are final. Any unused sessions WILL NOT be

refunded. I promise you will get out of the coaching session what you put into it.

Rescheduling Appointments: Please give me at least 24 hours advance notice to reschedule. Any

appointments rescheduled less than 24 hours advanced notice will result in a $50 no-show fee. Should there be

any extenuating circumstances (i.e. sickness, family emergency), the fee will be waived.

Confidentiality Policy: I will keep completely confidential all matters discussed, facts revealed, and

the nature and content of all documents you have provided to me or that I generate (within Anita

Birdwell Financial Coach, LLC). I will reveal such matters only upon receipt of a written authorization

and clear, verifiable instructions from you telling me exactly what I may reveal.
